District 622 has a continuum of services available to meet the needs of all students with disabilities who are eligible for and need special education services and related services. Placements occur after a student has been determined to be eligible for services in at least one primary area of disability as demonstrated in the special education Evaluation Report (ER).

Homebound Procedures

  1. Homebound Instruction Procedures- 9/6/2006 Memo
  2. Application for Homebound Instruction- Completed when the building administrator is requesting homebound instruction.
  3. Application for Hospital Instruction- Completed when the Physician is requesting homebound instruction.
  4. Assignment of Teachers for Students on Homebound Instruction-Guidelines for administrators to follow when assigning teachers to work with students on homebound instruction.
