Chapter 10-  Student Records

Here users can access information on the maintenance and storage of documents related to the education of students with disabilities. School District 622 permits personally identifiable information to be collected, maintained, or used as per district school Policy 515.


  1. Chapter 10 Records Retention for Students with Disabilities-Outlines procedures for records transfers, parent access, retention and destruction schedule.
  2. File Transfer Documents-  Form A and Form B
  3. Due Process File Organization Checklist- Assists with student record organization.
  4. Notice of Additional Records- This notice is placed in the studen'ts cumulative file since additional records for the student are maintained in another location in the district.
