Schools are required to review all possible public and private funding sources that might pay for health related services identified on your child's Individual Family Service Plan (IFSP) or Individual Education Plan (IEP). The District requires your approval to ask your Private Health Plan if services are covered. Services that are not covered may then be billed to MA/MNCR if appropriate. MA/MNCR allows services to be billed without approval from parents/guardians. They do however, require permission to share your child’s health records with physicians, the State health programs, and individuals involved in the billing process. If you agree to share this information, the district will be responsible for all follow-up paper work. Parents/guardians will not incur any direct out of pocket expenses under any of these circumstances. Please review the forms below.
